Accepting and Removing Patients from Our List

We can only accept new patients who live in our practice area.

You will need to come to the surgery during the following hours to register in person. 

Monday 10am - 3pm

Tuesday 10am - 3pm

Wednesday 10am - 1pm and 2pm - 3pm

Thursday 10am - 3pm

Friday 10am - 3pm

Please bring your medical card with you if you have one and some recent proof of address with your name on it, such as a utility bill dated within the last 3 months. If you do not have your medical card please ask your previous surgery for your NHS number.  We will ask you to complete your registration details and a health check questionnaire.

All newly registering patients over the age of 40 will be booked to see the healthcare assistant for a new patient health check.  This will make sure our records are accurate and all routine checks to promote your health and wellbeing (such as blood pressure/smear tests/mammograms) are up to date.

If you are registering children under 5 years old, please let us know what childhood immunisations they have received.

If you move out of our area please let reception know as we may be able to keep you registered, depending on your new address, otherwise we will ask you to re-register with a more local doctor. In exceptional circumstances, we reserve the right to remove any patient from our list.
